Please provide us with all necessary insurance information at your initial visit. Co-pays or deductibles are due at the time of service.
How much time is involved?
Allow 1 and 1/2 hours for your first appointment. There will be some paper work, then an evaluation, treatment and time to answer any questions you may have. After your first visit your appointments will last about 45-90 minutes depending on your treatment.
How does my doctor know my progress?
After your first appointment a written evaluation will be sent to your doctor with a proposed treatment plan. Progress notes will be sent prior to your next doctor visit.
How is billing handled?
OSPT is Medicare certified and we contract with most insurance carriers. As a courtesy we will bill your carrier directly.
What if need to change or cancel my appointment?
Please let us know 24 hours beforehand so we can try to give your spot to another patient. We charge a fee for appointments cancelled less than 24 hours in advance that we are unable to fill.
